General Information: Athletes must fill out all information on the front and back of the entry form, and mail it to the address indicated for the athlete's region of residency (see county listing below) or bring it to the tryout.

All candidates must attend and actively participate in the tryout process (dates listed below).  Unless otherwise noted, players need only attend one tryout date listed for their region.

Athletes selected for the final team will be provided with a competition jersey, and Games' sweat suit. Teams must provide their own equipment (pads/helmet/sticks, etc), pants, transportation, lodging and meals in Lake Placid. There is a $10 entry fee for those players officially named to the region roster, payable online as indicated below.

Eligibility:
Athletes must meet all of the following requirements to participate in Ice Hockey for the Winter Games:
- Athlete must be at least 18 years of age as of November 1, 2009; 
- Athlete must be a permanent resident of New York State or be a full-time student at a New York State school as of November 1, 2009.
- All candidates must attend and actively participate in the tryout process.
- All finalists must present photo ID showing NY State address, or current college photo ID.
- All candidates must adhere to the ESG Winter Rules as posted on the ESG Website.

The athlete's region of residency is the county in which the athlete's permanent mailing address is located or the county where the athlete attends school. Athletes may tryout for only one region.  Athletes may apply to try out in their region of residency or in the region where they attend school, but not both.  Athletes who reside in states other than NY who are not full-time students in NY State but who are employed within New York State are not eligible for the Games.

Registration and Entry Fee:  Skaters selected to their regional roster must confirm their intention to play in Lake Placid by registering online and paying the entry fee of $10 at the Winter Games registration page.  The link to the online registration page will be opened after January 15th, so please check back.
For the 2010 Winter Games, the online entry deadline is 11pm, February 17, 2010.  Rostered players who do not register and pay the entry fee by the deadline will be removed from the roster and will not be permitted to play at the Games.

Photo ID (government or school issued) and proof of NYS Residency or current Student Photo ID will be required at the tryout and at finals registration in Lake Placid.  Players will be required to show proof of NYS residency or school registration at finals registration if the Photo ID is not a NYS Drivers License or non-driver ID showing a NYS address.

Team: A team of 18 skaters plus two goalies will be selected to represent each regional team at the Games, to be held in Lake Placid February 19 - 21, 2010.  Rosters will be announced by February 3, 2010.
